 Day Habilitation - Provides assitance with learning, retaining, and improving in self-help, socialization, and adaptive skills. The service occurs in a place other than the individual's home.
MR Personal Care Service - Assists consumerwith eating, bathing, dressing, personal hygiene, and activities of daily living.
Respite - Provides periodic relief for a family or primary caregiver onan interim basis. This service can be used to meet a wide range of needs, including family emergencies, planned vacation, relief from daily responsibilty and stress of caring for someone to allow time for caregivers to shop, run errands and other tasks. Respite can be provided in the care provider's home, in the community, or in the respite provider's home (if certain provisions are met).
Family Training - Provides training to families of individuals with disabilities by CWI staff on specific issues related to the consumer. For example, family training may include communication techniques, structured scheduling, and behavioral techniques.
Supported living : Periodic & Residential - Promotes the individuals's learning, retaining, and improving in self-help skills. Service is provided in the community, the individual's own home, his/her family home, or the supported living living provider's home. Someof our consumer's reside in the home with their supported living provider. |
